Community Service Agency Approval Form

ASAP participants who are assigned community service work are given a list of agencies in the Rockingham-Harrisonburg area that are pre-approved.  We encourage particpants to do their community service at one of the agencies on the list.  Occasionally, someone referred to Rockingham/Harrisonburg ASAP lives in another jurisdiction and requests completing their community service work in another area.  Any agency not on the pre-approved list must be approved in writing by an ASAP case manager or the program director.   Generally, only non-profit or governmental agencies are approved.  

To request approval of an agency not on the pre-approved list, please download the form below and hand it in to your case manager.  Please do not begin work until you receive written approval of the agency from Rockingham/Harrisonburg ASAP.
